I suppose things could get non-trivial if you'd want to do it with the glyph positioning alone. Probably doable, yes, but it could as well turn out that it would be much easier to simply substitute the more common numerals with ligatures made of composite glyphs instead of setting up a chain of several contextual lookups or whatever. It all depends on how precise you'd want your layout to be and how many edge cases you'd want to support.
