I noticed another that the character U+E0E7 noteheadCircledDoubleWhole is a circled regular breve, but U+E0EB noteheadCircledDoubleWholeLarge is a circled square breve. Is there a reason for this?
If I opt to have a regular breve in both composites, would it be most appropriate to create an alternate, or could I just put the 'regular breve in large circle’ at the regular code point (U+E0EB?

There are no plans to do this at present, no. The way I have typically
handled this kind of thing in Bravura is to create stylistic alternates
for key glyphs that are optical variants, so should the need arise for
vulgar fractions not permitted by the current set of digits, I would
anticipate adding stylistic alternates for the remaining digits.
You're correct that if you want to create additional digits that are not
included in the core SMuFL character set, you should add them in the
optional glyphs range.
