As I understand it, the way arbitrary fractions are typically handled in
OpenType fonts (beyond the basic vulgar fractions of 1/4, 1/2, 1/3, etc.)
is to include two sets of numerals appropriately scaled and positioned
relative to each other, one for the numerators, and the other for the
denominators. These are then combined using the fraction layout feature.

I would suggest that this is the approach you take. In addition to
creating the full fraction layout feature, you could also consider
creating ligatures for common fractions, so that applications that have
ligature support but not support for the fraction layout feature can
access some common combinations more directly.
