To me, tenuto-marcato is quite common (Bartok, Debussy etc.), and I also was surprised not to find it in SMuFL.

For what it is worth, I did include it November2 (see doc. p.75 http://www.klemm-music.de/notation/november2/en/presentation/November_2.0_Presentation_Engl.pdf)
(as well as less common marcato-tenuto, marcato-staccatissimo, marcato-tenuto-staccato, etc.)
