There's no requirement that the glyphs to be combined require the font to
be used at the same point size. In general I am not in favour of including
duplicate glyphs in the standard that are identical except in size (as has
been suggested for e.g. clefs to be used as clef changes).
Perhaps it would be useful for me to clarify in the implementation notes
that correct appearance may require the usage of the font at a different
point size (perhaps expressed as a scale factor, e.g. "use the font at a
point size 20% smaller" or whatever) to achieve the right appearance.
However, since the standard actually includes slots for properly designed
octave clefs, I wonder whether scoring programs that implement the
standard will have any real reason to create octave clefs as composite
symbols anyway.
