I can absolutely assure you that a commercial music publisher will say that this sort of detail is a real problem, and they increasingly rely on the web as a delivery medium.
But if we can rely on the following, we'll be in good shape:
1. As Daniel said, require font authors to *also* provide the alternate TAB clef at an explicit code point, and
2. Supply metadata outside the font which specifies that this particular stylistic alternate resides at the code point in question.
